Helping community based therapists assist in transitions
People with developmental disabilities are hospitalized for many reasons, not just aspiration. The requirements of feeding in the hospital are different than what is found in the community. Often there is a downgrade in diet level which follows the client back into the community.
Participants will learn considerations of patient needs when hospitalized that can vary from community feeding programs. Learn about acute care, medical decisions that affect clients treatment and feeding changes that occur due to hospitalization. Learn how to set up communications between medical and community based professionals and work across professions to transition the individual back into the community successfully. There will be discussion and ideas for planning and treatment to help individuals return to baseline once they have returned to the community.
Objectives Participants will:
1. Understand the differences between acute care assessments and those in the community
2. Maximizing communication between medical and community based professionals
3. Working across professions to transition back to the community
4. Plan and implement treatment after discharge to help the individual return to baseline
Presenters
Anna Ortiz is a native New Mexican dedicated to providing excellent care in the medical setting. Anna obtained her Bachelor’s degree from Eastern New Mexico University and earned her Master's degree in Speech Pathology from Northern Arizona University in 2003. She has been employed with University of New Mexico Hospital since May 2004 working primarily with adults in the acute care setting. She was an adjunct professor for the UNM Speech and Hearing Sciences Center in 2008 where she taught the Adult Neurogenic Language course. Her areas of interest include evaluation and treatment of swallowing disorders, tracheostomy management, and providing mentorship and training to future SLPs.
Sydney Danziger has a master’s in speech-language pathology degree from UNM and has 20 years of experience with a primary focus in the treatment of dysphagia and cognitive rehabilitation. Sydney currently works in a variety of settings including UNM Hospital. She has helped establish UNM Hospital’s Special Needs Swallow Clinic and has performed many videos for individuals who are served through the DD Waiver. She also provides speech services to individuals of all ages on the waiver through KidPower Therapy Associates. Sydney has experience working in all areas of patient care, school based and private practice settings. Ms. Danziger is NDT certified. She was the first person in New Mexico to obtain the CPCRT. Ms. Danziger chaired the NM state licensure board for many years. She remains an active member of many organizations including society for Cognitive Rehabilitation and ACRM BI-ISG.
